Description: ICD-9-CM was developed to code and classify morbidity data from the inpatient and outpatient records, physician offices, and most National Center for Health Statistics (NCHS) surveys. As of October 1, 2015, Department of Health and Human Services (HHS) mandated that all entities covered by the Health Insurance Portability and Accountability Act (HIPAA) must transition to a new set of codes for electronic health care transactions in the form of International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) and International Classification of Diseases, Tenth Revision, Procedure Coding System (ICD-10-PCS). • a tabular list containing a numerical list of the disease code numbers;
• an alphabetical index to the disease entries; and
