Combine what was done in #92 and #93.
It was possible to format regions inside language buffers, and it was possible to format whole code blocks inside Org buffers, but it wasn't possible to format regions inside code blocks inside Org buffers.
That resulted in an inconsistency where only one of the menu items was going to be functional inside an Org buffer that had
