Update instructions to the new web site. Remove Australia and China cost. We don't use the premium network any longer, which costs differently for Australia and China.

The guide is moving to a dedicated site. This PR updates the link to the guide.
This PR is supposed to add a troubleshooting option for Google Cloud Nightscout as well.
The cost for Australia was checked by 2 individuals in Australia and they reported the cost was 0 or a few cents a month the same as in North America or Europe.
We are not using the premium network tier any longer and it is possible that is why the cost is not higher in Australia.
Assuming there is access to Google from China, there is no reason the cost will be any different considering.
