Oh, also, the middleware is breaking ActiveStorage redirects, because for those, the response body is a frozen string (accompanying a redirect status code), so attempting to insert generates an error. So I've added a fix for that too. |

Fix for #40 and also update documentation with correct way of configuring middleware.
