Bumped minor version for plugins.jquery.com

No other reason than plugins.jquery.com has got stuck on an earlier, erroneous, version and I can't see how to bump 1.2.1 over to that site.

There are so many places the version number needs to be updated, it is easy to miss one.

Test function now available

It is now possible to see a list of all fields that will be posted when the form is submitted, and this is useful for testing. Some corrections also included (e.g. fields without names are not posted, only one submit button is posted).
