Update governance documentation.

Was Is
N/A (Unclarified) PMC Chair shall be responsible for approving changes which do not impact the specifications
N/A Dissenting PMC members are encouraged to document their dissent
PMC consensus achieved before implementation PMC approval consensus achieved after final PR is submitted; PMC may disapprove at any time in the process
All PMC representatives must "approve" the pull request PMC Chair may document approval consensus captured via other channels
