Replace your ddraw.ini file in Diablo II\ProjectD2\ddraw.ini with the file supplied.

Copy the 4xScaleHQ.glsl into Diablo II\ProjectD2\Shaders.

Open Launcher for PD2, Click Options, Uncheck Windowed Mode.

You should be all set!
