The phtml does not render error after clicking NO.

Can you do a screencast, by any chance? I'm not sure I understand what you're trying to detail.

Of what i remember. The UI didn't respond appropriately whenever the user denied the authorization request. It should render the reason stated in the url.

Unfortunately I moved away from agility atm.
