Unnecessary code-block escape characters in summary/report #52

When generating the summary/report, the following line leads to faulty code-blocks:

vscode-code-annotation/src/reporting.ts

Line 26 in af81e3d

result += '```\n';

I might not understand the desired result, but in my case the code-block starts with the notes file name and ends before the actually annotated code.
