First pass at conversion to GitHub wiki • Move README.md → Home.md so it is the wiki index page. • Capitalize filenames and remove initial first-level headers since wiki page titles are derived from their filenames. • Rename some Markdown files in subdirectories since all pages in a GitHub wiki are treated as if in the top dir, regardless of where their files are located. • Adjust inter-document links to omit the .md part of filenames since it is not in the page URL.
