Audit Rouge use in Jekyll/middleman/other gems.

It has been a while since I've looked, but there are many consumers of Rouge which use the formatting layer improperly, for example resorting to post-processing or misusing the Rouge::Formatters namespace. I plan to audit these and submit PRs where appropriate.
