Integrate visa letter requests into conference system #130

Right now the workflow for a visa letter request is:

website asks people to submit an email with information
someone needs to confirm that the person has a ticket
coordination in team needs to happen who is going to write the letter
send letter
inform team (possibly separate email from the visa letter email)

This could be integrated into the conference system:

offer a Django form which asks the information, and fills in details from the registration
already checks that the person has a valid registration
sends an email with the request to the conference contact
and/or creates a ticket (see Integrate GitHub Project (Kanban) boards into conference system #115)
a ticket can be assigned, and once it's closed no further action is required
