This can be SQL or Opserver - can please you hit download on the plan and post it as a Gist so I can take a look?

Okay this will take some time (possibly part of re-writing all of the query plan to HTML bits), it's an artifact of many plan trees in a single plan. I don't think the original plugin handles this very well at all, and it also creates issues like the missing lines in your picture.
