Remove CHAPI Registration as Part of App Install

In reference to #536

We're going to take the the CHAPI registration out of the install flow. It will still be in the settings area. From there we can work on the user experience and consider how/when to introduce it to users.

In the future we may make this a feature flag but for now, please exclude CHAPI registration as part of the install flow.

We will keep #536 open and work on this from the registration link in settings.
