Inferno test failed when resource has Reference element but without Reference.reference sub element #589

When certain Reference type elements returned by the system under test do not have Reference.reference populated, Inferno tests provide a system error. While perhaps unusual, Reference.reference does not have to populated as long as identifier, display or extension sub-elements are populated.

This use case should be handled by ignoring such Reference/Resource instead of a hard failure
