Improve Navigation in Metadata Editor: Add Table of Contents (ToC) to left hand side

As a user I want to be able to quickly navigate to areas of the metadata editor. I imagine it would be useful to have a compact overview in the form of a table of contents with indentation and clickable links on the left hand side. The TOC could just show an outline of elements or could be both elements and fields.
