Clarify the README

The README of the next-gen repo and the melt-ui repo start exactly the same, which makes it confusing which repo you're currently looking at. I think the the README here should be titled melt next-gen or sth like that and also explain the differences. It would also be nice to display the NPM package names in both READMEs for clarity and to cross-link both READMEs.
