Add checklists as input values

Hi there, I want to know if there's any way to add an input value that requests for a checkbox type of input.
I need the user to check all "tests" that they need to request, so they are all included on a list in the body of the email.

Some of our products need just one test, and others need more, depending on the product's features. So I want the input value to ask for checkboxes like this:
□ Washing test
□ Weight test
□ Abrasion test

So, for example, if I check Abrasion test only, the email will look like this:

_Good morning, I need the following tests to be perfomed on this product:
Abrasion test
