Team avatars for renamed teams show incorrect letter

Steps to Reproduce

1. Start with any team that was initially called Foo, but is now called Bar
2. In various parts of the Sentry interface where teams/assignments are shown, the team and its avatar will display as [F] #Bar. Mousing over the F avatar will show foo.

Expected Result

It's expected that the icon's initial matches the first letter of the team

Actual Result

We see the avatar's letter match the original name of the team
