Improve "contact details in footer" implementation to automatically include url in issue body

This is a follow in issue from #336 so that we could release the MVP of the feature without delaying it.

Essentially we want to improve the implementation from #337 to include the "submitted from URL" in the body of the issue if possible.

If we do this we need to be careful not to override any default issue template that might be there on the repo already 🤔 if this is not possible then we should discuss the trade-off of resetting the default issue template with the link that they pushed the "create issue" link on 👍
