Improve the layout and copy for the analysis request form #60

The layout and copy for the analysis request form need to be improved to communicate why we're collecting this information, what's going to happen after "analyze" is clicked, and better give some insight into why the fields are ordered the way they. I'm thinking it'll look something like this:

What do you want to analyze? This needs to be a URL for accessing a publically available git repository, such as the ones hosted on GitHub or BitBucket.

How should we contact you when the analysis is finished? The analysis can take a few minutes to run (up to 20 minutes on larger projects with a lot of history), so we'll email you when it's done.

What should we call you? <name field>
What email address should we use? <email field>
