associated token account failure. - Error Number: 3012. Error Message: The program expected this account to be already initialized. #20

No idea where to proceed.I had this attached to a different issue, but since it was from a while back and slightly different, I thought I should just start a new issue.

I occasionally can get a transaction to send, but have never been able to create associated token account. I get the same error always.

Attempt 5 to create associated token account failed: Cannot send a request, as the client has been closed.
Max retries reached. Unable to create associated token account.
Buy transaction failed.

I keep getting several failed attempts ,but occasionally I get a transaction sent, with this error:
