I know there is another thread with two possible solutions to this but I stumbled across a much easier solution while looking up how to uninstall another Mac OS equalizer.
Instructions:
Uninstall eqMac using the eqMac uninstaller
Restart your Mac
In Finder navigate to the following folder: Macintosh HD/Library/Audio/Plug-Ins/HAL
Move the eqMac folder/directory to Trash (enter your password)
Empty trash
Restart Mac
This should completely uninstall the eqMac application and eqMac driver from your Mac. I hope this helps.
